Complete All Required Fields
Ensure all required fields are filled out accurately. Missing or incorrect information may delay processing.
Provide Your Information
Full Name: Enter your full name.
Address: Enter the address of the home for which you’re submitting the warranty claim.
Phone Number: Provide a valid contact number.
Email: Use a working email address to receive confirmation of your submission.
Describe the Warranty Issue
Clearly describe the issue you’re experiencing.
Attach Photos of the Issue
Accepted File Types: jpg, jpeg, png.
File Size Limit: Each photo must not exceed 10 MB.
Maximum Number of Photos: You can attach up to 5 photos.
Roof/Plumbing/HVAC Confirmation
Answer the final required question: “Please check this flag if you are reporting a roof leak, plumbing leak, or HVAC not working at all.” Select either “Yes” or “No.”
Submit the Form
Once submitted, you will receive a confirmation email acknowledging receipt of your warranty form.
